People close to Joe Biden say the former president remains active as he undergoes treatment for an aggressive form of prostate cancer, though some have noticed signs of increased fatigue in recent weeks.

According to Fox News, several friends who have spoken with Biden in recent months told The Washington Post they have not received detailed updates on his condition. 

They said that during conversations, he has at times seemed more tired, which they attributed to the strain of the disease and ongoing treatment.

Others offered a more upbeat assessment. One person described Biden as active and engaged, adding that he remains “encouraged and positive about his prognosis given his positive response to treatment.”

Aides have also maintained that the 83-year-old is continuing to work, including writing a memoir and preparing for public appearances. Biden is expected to travel to South Carolina on Feb. 27 to mark the sixth anniversary of his primary victory in the state.

Biden’s office disclosed in 2025 that he had been diagnosed with stage 4 prostate cancer that had metastasized to the bone. Medical experts consider that form of the disease incurable. 

At the time, his cancer was assigned a Gleason score of 9, indicating one of the most aggressive classifications and a higher likelihood of rapid spread.

His treatment has included radiation therapy at Penn Medicine Radiation Oncology in Philadelphia, along with hormone therapy administered in pill form.

On Oct. 20, 2025, Biden marked the completion of his radiation course by ringing a ceremonial bell, a moment his daughter Ashley described as a small celebration in his fight against the illness.

He has continued to appear at major events despite the diagnosis. In April 2025, Biden attended the funeral of Pope Francis. 

Later that year he was present at the funeral of former vice president Dick Cheney, and in January, he attended Tatiana Schlossberg’s funeral.

Friends and aides say the combination of medical treatment and public activity has contributed to fluctuations in his energy, but they emphasize that he remains focused on his work and upcoming engagements as he continues treatment.
