Es causado por la precipitación de minerales de la saliva y el fluido crevicular gingival (FCG) en la placa sobre los dientes. Este proceso de precipitación mata las células bacterianas dentro de la placa dental, pero la superficie rugosa y endurecida que se forma proporciona una superficie ideal para la formación adicional de placa. Esto lleva a la acumulación de cálculo, que compromete la salud de la encía. El cálculo puede formarse tanto a lo largo de la línea de las encías, donde se refiere como supragingival, como dentro del estrecho surco que existe entre los dientes y la encía, donde se refiere como subgingival.
