O rio tem 206 kilometres (128 mi) de comprimento, e sua bacia de drenagem cobre 4,870 square kilometres (1,880 mi2). Ele nasce no Chastinsky District, perto da vila de Pikhtovka. Em seguida, flui pelo Bolshesosnovsky District de Perm Krai e pelo Votkinsky District de Udmurtia. Sua foz fica a jusante da barragem da Votkinsk Hydroelectric Station. O Siva congela na segunda metade de outubro e permanece coberto de gelo até abril.
