Blue Cross analysis links AI coding to $942M in added costs

A Blue Cross Blue Shield Association analysis says hospitals’ growing use of AI-assisted coding and documentation tools coincided with more inpatient stays being billed as medically complex, increasing costs for its member plans by an estimated $942 million over two years. The share of stays classified as complex rose from 37% to 40%, with $653 million of the added costs attributed to secondary diagnoses that moved claims into higher-reimbursement categories. The association says it found no corresponding change in care to explain the coding increase, raising concerns that patients, employers and taxpayers could ultimately face higher costs. A separate analysis cited in coverage of the report found especially steep increases in complex coding at hospitals with the fastest growth, including maternity diagnoses that rose despite flat transfusion rates. Hospitals and industry groups argue that AI tools can help counter claim denials, payment delays and administrative burdens, while the findings have renewed debate over whether the higher bills reflect appropriate documentation or unnecessary upcoding.
