Iowa Medicaid Fraud Unit Reports 37 Convictions at Lowest National Cost

Iowa’s Medicaid Fraud Elimination Task Force is reviewing ways to prevent and detect fraud, with recommendations due to Gov. Kim Reynolds by Oct. 29, 2026; no specific proposals have been adopted yet. The state’s nine-member Medicaid Fraud Control Unit reported 37 convictions in federal fiscal year 2025, ranking 10th among 53 units, while operating on $1.3 million and recording the lowest cost per conviction nationally. The unit also reported recovering more than $3.9 million through August 2026, a 338% return on operating expenses. Its director says stronger collaboration among state agencies and managed-care organizations, along with greater use of Iowa’s False Claims Act, could increase referrals and recoveries. Former state agency director Dean Lerner and other critics say the unit needs more staffing and funding, despite its reported efficiency.
